Use this for a recurring manager–teammate conversation. Start with the teammate’s priorities, then agree what support would help. This fictional example concerns a blocked research project; it is not a performance assessment.

Sam’s priority is finishing the onboarding study. Recruiting is taking longer than expected.

Sam completed the interview guide. Recruiting three more participants is still open.

Sam needs an introduction to the customer success team to reach more participants.

Jordan suggested testing the interview guide with a colleague first. No development goal was agreed in this conversation.

Agreed follow-up

Jordan will introduce Sam to customer success by Wednesday. Sam will run a practice interview on Thursday.

Read the last note together and bring forward only commitments that still matter. Add a personal agenda without turning it into a required disclosure of private information.

During · leave room for support

Discuss the obstacle behind the update. Ask what help is useful and whether the teammate wants advice, a decision, or simply space to work through a problem.

After · agree access and follow-up

Confirm the next steps and who may read the note. Keep confidential topics out of broadly shared team folders, and revisit commitments without rewriting the original record.

Should a one-on-one include project status?

A little context helps, but status should not consume the whole conversation. Use the note to uncover blockers, offer support, discuss feedback, and preserve agreements that need attention between meetings.

Who should own the notes?

Agree that together. A shared working note can help both people follow through, while sensitive topics may need a different access policy. This template does not replace your organization’s personnel-record guidance.
